($2,500.00 sign on bonus - 1st payout 90 days, 2nd payout 180 days)We are seeking a compassionate and skilled Nurse (RN or LPN) in San Carlos, AZ, to provide patient care in accordance with a physician-established care plan.This is a 1099 contract opportunity, with care provided at the patient’s residence. The patient requires 14 hours of WEEKLY nursing care.Once patients are accepted into the program, there is no expiration for this benefit, allowing our caregivers to build lasting relationships with their patients for years to come.In this role, you will advocate for the patients and work one-on-one to deliver the care they need. You will collaborate with CNAs, HHAs, Case Managers, and the patient’s physicians.Offers are contingent upon passing a background check.#INDLICResponsibilities:Execute physician prescribed plans of careComplete compliant documentation of the care provided in the system of recordManage and administer prescribed medication, treatment, and therapiesConduct patient assessments, coordination of carePerform various activities associated with daily livingMonitor vitals and GI intake and outputAssess skin integrity and administer wound careMonitor for changes in the patient’s condition such as weight loss/gain, self-care abilities, and indicators of disease progressionCommunicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patientsProvide education and training to the patient, their family, and/or the Home Health AideWork with patients to be done one-on-one in the patients home to provide for a comfortable environmentCollaborate with and supervise Certified Nursing Assistants and Personal Care AttendantsPerform other personal care services as necessary to meet the patient’s needsMinimum Qualifications:Education, licensing, and certification: graduate of an accredited college or university, Associate or Baccalaureate School of NursingCurrent State or Compact License as a Registered Nurse or Licensed Practical NurseCurrent CPR certificationTB test and physical examinationMaintain required insuranceGood interpersonal skillsHand-eye coordination and manual dexterityAbility to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patientsAbility to operate under stressful conditions and make quick decisionsMust have dependable transportation and be willing to commute to the patient’s homeAbility to provide basic medical management up to medically fragile end of life carePreferred Qualifications:1+ years of experience as an RN/LPN in a clinical or home health settingAbility to assist with two person assistTo provide the exceptional care our patients deserve, we rely on a team of passionate, dedicated professionals.
