Pay:Salary based on experience plus superior benefits plan (PTO, paid holidays, HSA, medical, dental, vision, pension, and more)JOB DESCRIPTION: TheCare Manager / Social Worke r will provide care management services to a Medicaid program that helps older adults with disabilities receive care and support in their homes, instead of a nursing home. Duties include providing intake, assessment, and care management services to clients, including ongoing assessment for program eligibility, monitoring all aspects of care and client services (social and medical), and ensuring the needs of clients and caregivers are met through a collaborative process.REQUIREMENTS: OH Licensed Social Worker (LSW) in good standing At least one year of experience working with geriatric persons with disabilities/chronic conditions and LTSS Prior care management / case management experience and electronic documentation preferred Valid driver’s license with good record and reliable transportationAPPLY TODAY with an updated resume to be considered!
