Provides rehabilitative, skilled, and medically necessary treatment intervention to patients with physical functioning disorders under the supervision of a Physical Therapist. Administers various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices. Assesses patient progress and communicates with supervising Physical Therapist to adjust treatments accordingly. Reports effectiveness of treatment outcomes to the supervising Physical Therapist. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides input for discharge summaries of services provided based on regulatory and clinical requirements. Documents all necessary information in the electronic documentation system and maintains patient records according to policy. Accurately records treatment minutes for services provided in accordance with all regulatory and clinical requirements. Participates in required meetings, including patient care conferences and utilization review meetings. Participates in required training and conferences. Works with supervising Physical Therapist to obtain necessary equipment or supplies per regulatory and clinical requirements. Supervises and supports students under the guidance of the Physical Therapist. Adheres to practice acts, code of conduct, and ethics during professional practice as a Physical Therapist Assistant. Maintains active State Licensure when applicable.
