Pharmacist Job Title: Pharmacist Profession: Pharmacy Specialty: Clinical Pharmacy Duration: 26 weeks Shift: Night Hours per Shift: 10 Experience: Minimum of 2 years in acute care hospital License: Current pharmacist licensure in Florida Certifications: BLS and ACLS helpful but not required Must-Have: PharmD or Bachelor's degree with recent acute care hospital experience. Ability to accurately check and/or double check pediatric oral and intravenous solutions. Experience in pharmaceutical calculations and pharmacokinetics dosing of aminoglycosides and vancomycin. Experience in TPN calculations. Ability to resolve issues related to medication dispensing systems. Description: The clinical pharmacist shall practice pharmacy with the highest ethical standards. The pharmacist is responsible for safe, effective, evidence-based, and cost-effective medication selection and utilization. This role requires advanced knowledge, skills, and professional judgment in all aspects of pharmacy practice. The pharmacist serves as a patient advocate when planning and implementing pharmaceutical care within a multidisciplinary team. Collaboration with other healthcare professionals is essential. Adherence to all regulatory and medication safety guidelines is paramount. The pharmacist verifies computerized medication orders for accuracy and clarifies any unclear or potentially unsafe prescriptions. Medication profiles must be accurately transcribed to the patient's medical record. The pharmacist prepares and dispenses medications safely and accurately, adhering to appropriate guidelines. This includes sterile, chemotherapy (limited), and parenteral nutrition preparations. The pharmacist develops, assists, and executes drug therapy management protocols. Responsibilities include assessing pharmacokinetics and managing pain and antimicrobial stewardship. Controlled substances must be issued to patient care areas with proper documentation. The pharmacist is responsible for reporting adverse drug reactions and medication errors timely. Supervision of pharmacy support personnel is required, including verifying their daily activities. Participation in the performance appraisal of pharmacy support personnel is expected.
