Reporting to the Chief of Medicine, this position is integral to the medical center and includes a faculty appointment at a local university, allowing for involvement in academic pursuits and mentorship of future healthcare professionals.Key responsibilities include:Assessing and caring for both new and returning patients requiring palliative care.Conducting consultations and formulating treatment plans.Providing follow-up care for both inpatient and outpatient settings.Participating in multidisciplinary team meetings, quality improvement projects, and educational initiatives.Veteran-Centered CareIn this role, you will:Deliver comprehensive palliative care focused on symptom management, enhancing quality of life, patient education, and navigating serious illnesses.Encourage shared decision-making with veterans and their families to achieve the best health outcomes.Perform consultations and create personalized treatment plans for patients in need of palliative care.Manage ongoing care, including symptom control and pain management.Provide both outpatient and inpatient palliative care services, including emergency consultations as necessary.Quality Improvement InitiativesEngage in continuous quality improvement efforts to enhance care delivery by:Utilizing various quality measurement tools to assess and improve clinical performance and patient outcomes.Serving as a mentor and role model for fellow healthcare providers within the palliative care team.Participating in educational activities to support residents, students, and other trainees.Healthcare CoordinationCoordinate care for veterans by:Integrating specialty care, mental health services, and other ancillary services to ensure comprehensive healthcare delivery.Collaborating with other healthcare professionals to develop tailored treatment plans for veterans.Impactful WorkProvide thorough, patient-centered palliative care to the veteran population, ensuring their health and well-being.
