Case Management Specialist - Aging & Long Term Care

This role is ideal for a skilled professional looking to deepen their expertise while making a measurable impact in the community.As The Case Management Professional, You Will **Ensure** the completion and accuracy of client files, documenting all case management activities in coordination with clients, families, service providers, and state and local agencies. **Conduct** comprehensive screenings and assessments and develop individualized care plans that identify client strengths, needs, and available support systems. **Coordinate** access to services by providing referrals and collaborating with interdisciplinary teams to ensure effective and appropriate service delivery. **Manage** client eligibility processes, including screening for program qualification and tracking presumptive eligibility status. **Monitor** program reports and track completion of new and ongoing case management tasks to ensure compliance with established deadlines. **Generate** and review data reports for multiple programs as requested to support program operations and oversight. **Provide** consultation and problem-solving support to staff as needed, particularly when the supervisor is unavailable.**[What you bring:]{.underline}****Educational/Professional Experience**Bachelor's degree from a college or university accredited by an agency recognized by the U.S. Department of Education in social work, Human Services, Psychology, or a related field AND two years of paid, on-the-job social service experience.
