Role Description This is a full-time remote role for a Licensed Practical Nurse (LPN). The LPN will be responsible for providing quality patient care, supporting field nurses, taking orders, writing orders, ordering wound care supplies for our different payers, collaborating with healthcare teams, and maintaining patient records. Additional responsibilities may include performing routine assessments, assisting with treatment plans, and educating patients and families about care procedures and health management. QualificationsHomecare Homebase R2 Experience required Licensure as a Licensed Practical Nurse (LPN) and adherence to state and federal regulationsStrong clinical skills, including patient care, medication administration, and health monitoringAttention to detail with experience in maintaining accurate patient records and conducting routine assessmentsEffective communication, interpersonal, and teamwork skills to collaborate with healthcare professionals, patients, and their familiesAbility to work independently in a remote healthcare settingBasic computer literacy for electronic health record (EHR) managementExperience in long-term care, home health, or a related healthcare setting is a plusCommitment to compassionate care and patient-focused service
