Pride Health is hiring a Travel Physical Therapist (Home Health/Hospice) to support our client’s medical facility based in Flossmoor, IL. This is a 3-month contract opportunity and a great way to work with a leading home health organization.Job Details:Job Name: Travel Physical TherapistLocation: Flossmoor, IL 60422Duration: 3-month ContractPayrate: $54 - $56/hr on W2Schedule: Monday - Friday; 8:00 AM - 4:30 PMJob Responsibilities:Conduct comprehensive in-home patient evaluations.Develop and implement individualized care plans. Provide physical therapy interventions to improve mobility, strength, balance, and pain management. Monitor patient progress and adjust treatment plans as needed. Maintain accurate, timely documentation including OASIS assessments.Educate patients and caregivers on exercises, fall prevention, and safety. Coordinate care with physicians, nurses, and interdisciplinary teams. Identify home safety risks and recommend modifications. Manage daily patient visits efficiently across the assigned territory. Ensure compliance with Medicare & regulatory standards Requirements:Graduate of an accredited Physical Therapy program (DPT or equivalent)Active, unrestricted Physical Therapist (PT) license in the state of practiceCurrent CPR/BLS certificationMinimum 1 year of clinical experience (home health experience preferred)Valid driver’s license and reliable transportationPreferred Qualifications:Experience with home health or community-based careEMR/EHR documentation experience, including OASISStrong time management and organizational skillsExperience with geriatrics and chronic disease managementAbility to build rapport with patients and caregivers in home settingsBilingual skills based on patient population needs Equal Employment OpportunityPride-Health is an equal opportunity employer.
