Occupational Therapist
Job Summary: The Occupational Therapist provides occupational therapy services to patients in their homes within the Medford, OR, and surrounding areas. This role involves assessing patient needs, developing treatment plans, and implementing interventions to improve patients' functional abilities and overall quality of life. The therapist is expected to manage their schedule efficiently, maintain accurate documentation, and collaborate with other healthcare professionals.Location:Medford, Oregon, United StatesResponsibilities:- Conduct patient assessments to determine occupational therapy needs.- Develop and implement individualized treatment plans.- Provide skilled occupational therapy interventions to improve functional abilities.- Educate patients and caregivers on therapeutic techniques and home exercises.- Document patient progress and maintain accurate records using EPIC/Oasis.- Collaborate with physicians, nurses, and other healthcare professionals.- Manage a caseload of patients, visiting up to 6 patients per day.- Ensure productivity standards are met (30 productivity points standard).- Maintain effective communication with the healthcare team.Required Skills & Certifications:- Active Occupational Therapist license.- Basic Life Support (BCLS) certification.- Minimum of 1 year of skilled home health experience.- Experience with Oasis documentation.- Must have own transportation and car insurance.- Primary series COVID vaccination in addition to one boosterPreferred Skills & Certifications:- Not specified.Special Considerations:- Traveler is expected to have their own transportation.- The system groups clients to minimize drive time, but occasional travel up to a 50+ mile radius may be required.- Facility can call off UP TO one shift per pay period (every two weeks)- No on-call requirements.- No weekend requirements.- Traveler will not float between like units.- RTO restrictions: Confirm with hiring manager.Scheduling:- 8-hour day shifts.- Shifts are determined and assigned by the facility.
