Your responsibilities include conducting assessments, developing and implementing treatment plans, coordinating services, and monitoring patient progress. You will collaborate with multidisciplinary teams and community agencies to facilitate seamless transitions and comprehensive care delivery, all within a supportive and structured schedule.Required Skills:Minimum Associate Degree in Nursing from an accredited institutionCurrent, unrestricted RN license from any stateCertification in one of the following: CCM, CDMS, CRRN, COHN/COHN-S, ACCC, CRC, RN-NCM, or CMCAt least one year of nursing experience post-graduationAbility to pass a government background check/security clearanceCurrent BLS Healthcare Provider certification from AHA or ARCStrong assessment, planning, coordination, and documentation skillsExcellent communication and interpersonal skillsNice to Have Skills:Experience with case management software and databasesKnowledge of community resources and social servicesFamiliarity with medical care criteria such as InterQualExperience working with military or veteran populationsAbility to participate in telehealth and virtual meetingsPreferred Education and Experience:Bachelor's Degree in Nursing or higher is advantageous but not mandatoryMore than one year of nursing experience, especially in case management or related fieldsPrior experience working within multidisciplinary teams or community health settingsOther Requirements:Work schedule: Monday to Friday, 8.5 hours per day, between 7:00 am and 5:30 pmNo weekends, holidays, on-call, or call-back dutiesAbility to travel or transfer patients regionally as neededWillingness to participate in teleconferences and team meetingsThis is a rewarding opportunity to advance your nursing career while contributing to vital patient care initiatives.
