Senior .NET Engineer - Health Tech (Europe) #2926About the RoleAs a Senior Back-End Engineer on the Platform Engineering team, you will play a critical role in architecting, migrating, and scaling the core engine powering Europe’s largest digital-first home care infrastructure. This position requires deep language mastery, production-tested architectural instincts, and a business-first mindset. You will design and operate event-driven, multi-tenant systems that handle real-time scheduling, finance, and care planning.You will implement Domain-Driven Design (DDD), execute zero-downtime legacy migrations via patterns like Strangler Fig, and heavily leverage AI-native development workflows (Cursor, Claude Code) to ship high-quality code at pace. Your work will directly impact millions of data points across a regulated health-tech ecosystem, requiring close collaboration with clinical, finance, and operations teams to translate complex real-world processes into highly auditable code.Most Important6+ years of experience in backend software engineering with a deep, production-level mastery of .NET and C# (owning the runtime and language, not just using it).Deep expertise in Event-Sourcing and CQRS architectures (handling schema evolution, replays, and projection failures in live environments).Extensive hands-on experience with Kafka or similar event-streaming technologies at high volume.Advanced experience with PostgreSQL at scale, including sophisticated multi-tenant data patterns.Proven track record of legacy migrations—specifically moving live production systems to new architectures without downtime.Strong proficiency in Domain-Driven Design (DDD) with the ability to strategically slice complex domains into clean bounded contexts.Cloud fluency within the AWS or GCP ecosystem, with a willingness to own infrastructure end-to-end (CI/CD pipelines, deployments, and observability).Mastery of AI-native engineering workflows (daily integration of tools like Cursor, Claude Code, or GitHub Copilot into your development lifecycle).Located in Europe with the availability to overlap core business hours with United Kingdom.Nice to HaveExperience working within highly regulated sectors (Health Tech, Social Care, FinTech, or Defense).Familiarity with healthcare standards and integrations (e.g., FHIR, SNOMED-CT, compliance auditing).Experience building LLM-powered features in production, including evals, drift monitoring, and AI observability.Practical knowledge of complex scheduling, routing, or constraint-solving algorithms at production scale.Experience utilizing tools like Akka.NET, EventStore, or Marten.Personal TraitsStrong ownership mindset with the ability to solve complex problems independently.Clear communicator who collaborates effectively across teams.Comfortable working in fast-paced, high-impact environments.You go and find out how real-world clinical and operational processes work before writing a line of code.You prioritize iterative delivery and shipping the smallest thing that works over perfection, and you adapt quickly when requirements pivot.You don't wait for a spec or an assignment—you see a structural vulnerability or feature gap and own the solution end-to-end.
