Our mission is to promote independence, dignity, and quality of life for the members we serve while maintaining the highest standards of compliance and care.Position OverviewEpic Nursing Services is seeking a Case Manager to support members enrolled in the Adult Foster Care (AFC) and Group Adult Foster Care (GAFC) programs.The Case Manager works directly with members, caregivers, and healthcare professionals to coordinate services, monitor care plans, and ensure that members receive person-centered support that meets MassHealth LTSS program requirements and CARF accreditation standards.This role involves regular member visits, care coordination, documentation, and collaboration with the clinical team.QualificationsBachelor's degree in Social Work, Human Services, Psychology, Public Health, Nursing, or a related fieldEquivalent experience in community-based healthcare or human services programs may be consideredExperience with MassHealth LTSS programs preferredExperience working in Adult Foster Care (AFC), Group Adult Foster Care (GAFC), PCA programs, or home care programs preferredStrong documentation and organizational skillsExcellent communication and interpersonal abilitiesAbility to manage multiple member cases simultaneouslyAbility to work independently in community settingsCultural competency working with diverse populationsValid driver's license and reliable transportationAbility to travel to member homes within assigned service areasAbility to maintain confidential records in accordance with HIPAA requirementsKey ResponsibilitiesConduct required member visits in accordance with AFC and GAFC program regulationsMonitor member health, safety, and overall well-being in the communityIdentify and report changes in member condition to the clinical teamCoordinate services between members, caregivers, healthcare providers, and community partnersAssist members in accessing community resources and support servicesSupport effective caregiver-member relationships and communicationCollaborate with the Registered Nurse to support person-centered care planningParticipate in service plan reviews and updates when member needs changeMaintain accurate and timely documentation in accordance with MassHealth LTSS AFC/GAFC regulations, Epic Nursing Services policies, and CARF standardsEnsure all case notes, member interactions, and service updates are documented appropriatelyParticipate in internal chart reviews, audits, and quality improvement activitiesSupport Epic Nursing Services' performance improvement and compliance initiativesAdvocate for the rights, dignity, and independence of members served Why Join Epic Nursing ServicesMission-driven organization focused on supporting individuals in the communityOpportunity to make a meaningful impact in members' livesCollaborative and supportive work environmentOpportunity to grow with a rapidly expanding healthcare organization
