Patient Care CoordinatorJoin our team as a Patient Care Coordinator with a focus on Chronic Disease Management. We're looking for a dedicated Patient Care Coordinator to join our Valley County Health System (VCHS) team. This is a full-time position working 5 days per week. In this role, you will:Provide education and support to patients living with chronic conditions such as diabetes, heart disease, COPD, and hypertensionDevelop individualized care plansMonitor patient progress and outcomesCollaborate with interdisciplinary healthcare teamsEmpower patients to build sustainable, healthy habitsProvide direct patient care within the medical clinicThe ideal candidate is compassionate, organized, able to be a self-starter, can work independently, and is committed to patient-centered care. Experience in care coordination, patient education, and chronic condition management is preferred. Applicants must have current Nebraska licensure as a Registered Nurse.
