Our mission is to empower clients to regain independence, recover their potential, and thrive in their communities.Role Description We are seeking aPhysical Therapistto join our team as anindependent contractorinSanta Maria, San Jose, San Luis Obispo, Santa Barbara, Sacramento,andStocktonarea. This role involves delivering high-quality, goal-orientedin-person and/or teleheath physical therapy servicesto clients in their homes and community environments. The Physical Therapist will conduct evaluations, develop treatment plans, and provide therapy sessions while collaborating with our interdisciplinary team to ensure comprehensive rehabilitation outcomes.Key Responsibilities Perform thorough patient evaluations and develop individualized treatment plans. Deliver in-home and community-based physical therapy sessions for clients with brain and spinal cord injuries. Collaborate with Occupational Therapists, Speech Therapists, and case management staff to coordinate patient care. Maintain accurate documentation of treatment sessions. Communicate effectively with clients, families, and the NeuroPraxis team usingHucufor case-related messaging. Ensure compliance with all professional, legal, and ethical standards of care.Qualifications Licensed Physical Therapist inCalifornia . Expertise in Physical Therapy and Rehabilitation with a focus on personalized treatment plans Master’s or Doctorate in Physical Therapy. Experience working with clients with brain and spinal cord injuries. Pediatric experience is a plus. Ability to work independently and as part of a multidisciplinary team. Experience in home health or community-based care is preferred. Spanish-speaking is preferred but not required. Willingness to travel to client locations as needed Compassionate and patient-centered approach to care Knowledge and experience in Medicine to support evidence-based therapeutic practicesWhy Join NeuroPraxis?
