DescriptionSchedule: 40 hours per week | 8am - 4:30pm | Monday - FridayDepartment: Home Care | PACE of Southwest Michigan | St. Joseph, MITravel Requirements: This role involves regular travel providing Home-Based Care.What You'll Do:The Home Care Participant Care Assistant (PCA) provides essential in-home care and assistance to participants enrolled in the PACE program. You will work closely with the Home Care Supervisor to deliver compassionate, person-centered care and support activities of daily living. From assisting with daily activities to providing comfort and companionship, your work will make a meaningful impact on the lives of those we serve.Key ResponsibilitiesProvide Hands-On Care in Their Home – Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting.Support Mealtime– Help participants enjoy their meals and stay hydrated for good health.Keep Things Moving – Support mobility, including turning, positioning, and safe transfers.Perform Vital Checks – Measure and record vital signs, weight, and intake/output.Maintain Comfort & Care – Support housekeeping tasks such as laundry, sweeping, and grocery shopping.Provide Responsive Support – Respond with kindness and compassion.Monitor with Watchful Eyes – Report any changes in conditions to the leadership ASAP.Demonstrate Respect & Professionalism – Maintain confidentiality while delivering top-notch care.The above is a summary of the position, it in no way states or implies that these are the only duties you will be required to perform. If selected for the position, you will receive a full job description.Skills, Knowledge and Expertise:One (1) year of prior experience with frail or elderly individuals, aiding with daily living activities. CNA certification, desired.
