Physical Therapy Assistant
Healthy Living at Home - Portland, LLC is in search for a professional and caring PHYSICAL THERAPIST ASSISTANT (PTA) to provide physical therapy services, assessment, direction and care to our Home Health patients in the greater Salem, CA metropolitan area.Position OverviewThe Physical Therapist Assistant (PTA), in conjunction with direction from the Physical Therapist and Physician Orders, evaluates, plans and administers comprehensive physical therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative physical therapy as prescribed by a physician.Essential Job FunctionsPerforms accurate and comprehensive documentation.Receives physician orders from physical therapist for certification and recertification of home physical therapy services.Ensures that changes in patient condition are communicated to other healthcare team members and staff in a timely, accurate, and thorough manner.Coordinates care planning and delivery with the physical therapist, patient and family/caregivers, other healthcare team members and staff to facilitate optimal patient outcomes.Educates patients, their families/caregivers and other staff as appropriate, in appropriate physical therapy interventions.Provides, physical therapy modalities based upon an ongoing assessment/reassessment of the patient’s clinical status by the physical therapist, in accordance with physician orders and in accordance with the standard of care for patient’s age.Assesses the patient for the need for durable medical equipment as well as other adaptive equipment needs.Provides physical therapy interventions in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.Identifies risk of injuries or threats to life and health in the patient’s environment, i.e., environmental/safety/mobility risks and implements appropriate interventions to reduce those risks.Completes and submits necessary documentation and verification of completed visitsParticipates in interdisciplinary team conferences in accordance Maintains current knowledge of community resources.Other related duties as assignedBasic QualificationsAssociate Degree in Physical Therapist Assistant (PTA) from a program approved by an accredited organizationLicensed to practice as Physical Therapist Assistant (PTA) within the state, if applicable.One (1) year or more experience as a Physical Therapist Assistant (PTA)Good verbal, written communication and organization skills.Possesses current CPR certification.Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. Preferred QualificationsTwo (2) years or more experience as a Physical Therapist Assistant (PTA) One (1) year or more experience as a Physical Therapist Assistant (PTA) in a home health communityPhysical RequirementsTypical functions of this role include standing, sitting and walking a majority of the time. There are occasions when kneeling, stooping, bending or reaching may be necessary.The employee must be able to lift and/or move up to 10 lbs. on a regular basis. There may be occasions in which the employee must be able to lift or move up to 40 lbs.Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.NOTE: This job description is not intended to be all-inclusive.
