This role is ideally suited to a senior nurse or ACP, and experience of working with this population is essential. It is part time hours 16 hours per week. It is initially offered as a fixed term 12 month contract, but if successful then there is scope to become permanent. You will join a team including a lead GP for CYP and a Lead ACP for Inequalities - with strong administrative support.Main duties of the jobYou will be working for all of the Practices within the Weymouth and Portland PCN - The Bridges Medical Centre, Cross Road Surgery, Wyke Regis & Lanehouse Medical Practice, Royal Manor Healthcare, Weymouth Bay Medical Practice.The Overall Aims Of The Role Are To:With support, identify the inequalities in health care provision and access to GP services for children and young people in the Weymouth and Portland PCN.
