Direct Service Provider (Respite & In-Home Support)
Overview
We are seeking a compassionate and reliable Direct Service Provider to deliver respite and in-home support services for an 18-year-old male client through the CDC+ (Consumer Directed Care Plus) program. This role focuses on promoting independence, ensuring safety, and supporting daily living needs in a home-based setting.
Responsibilities
Provide direct care and supervision in the home and community
Assist with daily living activities (e.g., personal care, meal preparation, light housekeeping)
Support social engagement and skill development
Follow individualized support plans and maintain a structured environment
Ensure the health, safety, and well-being of the client at all times
Communicate effectively with family members and care team as needed
Qualifications
Experience working with individuals through the Agency for Persons with Disabilities (APD) required
Ability to pass a Level 2 background screening (required)
Reliable transportation and punctuality
Strong communication and interpersonal skills
Patient, dependable, and able to work independently
Preferred:
Experience with adolescents or young adults with developmental disabilities
CPR/First Aid certification
Schedule:
25 to 30 hours a week.
