In this vital role, you will provide emotional support, companionship, and practical assistance to individuals receiving hospice care. Your presence and dedication will help create a comforting environment, ensuring patients feel valued, cared for, and respected during their final journey.This UNPAID position offers a rewarding opportunity to give back to the community while gaining valuable experience in patient-centered care.This opportunity is open to the entire state of CT, as we provide services to the whole state, but you will be paired with an individual within your geographic region for ease of service.The volunteer selection process at Affinity/Continuum begins with an application and an interview, where potential volunteers may express their personal goals, availability, interests and talents. Volunteers are carefully screened through a criminal background check, motor vehicle report, and personal references to ensure our patients are under the best care possible.ResponsibilitiesOffer companionship and emotional support to hospice patients, fostering a sense of comfort and connection.Assist with activities of daily living (ADLs), including grooming, errands, and light housekeeping tasks such as cleaning and laundry.Ensuring safety and adherence to HIPAA regulations.Help with meal preparationProvide transportation assistance for appointments or outings within the community when needed.Maintain detailed documentation of patient interactions and observations to support ongoing care efforts.Assist with office managementQualifications:MUST HAVE RELIABLE MEANS OF TRANSPORTATIONStrong communication skillsCompassionate attitude with a genuine desire to support patients through hospice care or home health servicesMust be reliable, adaptable, and able to work independently or as part of a team dedicated to providing exceptional hospice carePrior experience in home & community care, assisted living, nursing homes, or long-term care environments is highly desirableWork Location: In person
