Required Availability: Full-Time | Monday – Friday, 8:00 AM – 5:00 PM (No On-Call) Productivity Benchmark: 30 production points weekly. HOME HEALTH RN CASE MANAGER | SUMMARY AND HIGHLIGHTS: The Home Health RN Case Manager will serve as a patient advocate, collaborating with multidisciplinary teams to ensure clinical, functional, and fiscal goals are met. This role is responsible for conducting comprehensive assessments, developing individualized care plans, and ensuring patients transition smoothly between levels of care. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates HOME HEALTH RN CASE MANAGER | RESPONSIBILITIES: Assessment & Care Planning: Perform comprehensive assessments of high-risk members and collaborate with primary care providers to implement individualized, comprehensive care plans. Transition Coordination: Coordinate with physicians, social workers, and discharge planners to transition patients to the appropriate level of care. Member Support: Identify and implement non-medical supports (e.g., housing, transportation) to ensure compliance with treatment plans. Clinical Oversight: Reassess cost-efficient levels of care and identify opportunities for health promotion and illness prevention. Documentation: Maintain detailed records of clinical outcomes and interventions during the management process. Advocacy: Act as a patient advocate while strictly protecting privacy and confidentiality. HOME HEALTH RN CASE MANAGER | REQUIREMENTS: Education: Associate's or Bachelor's Degree in Nursing (ASN or BSN). Attendance: Commitment to mandatory attendance during the first 90 days. Licensure: Current, unrestricted Registered Nurse (RN) license in the state of New Jersey. Ability to analyze complex medical information; Case Management certification is highly preferred. Preferred Skills: Proficiency with Homecare Homebase (HCHB) and OASIS documentation. Ability to cover Passaic County (mileage reimbursement provided). Take the Next Step!
