The ideal candidate will partner with residents and collaborate with other healthcare providers to deliver physical therapy services aimed at maximizing functional abilities and enhancing independence in daily activities.Essential Duties and Responsibilities Promote and support the mission and values of our organization. Perform evaluative and therapeutic procedures under the guidance of a physician or accepted healthcare provider while adhering to safety protocols. Conduct resident evaluations in accordance with departmental protocols and develop individualized therapeutic plans that set functional goals and outline the frequency and duration of services. Engage residents and primary caregivers in discussions regarding the therapeutic plan of care, incorporating their input into therapy goals. Consult with referring physicians and healthcare team members about the plan of care and discharge needs of residents, attending relevant team meetings. Monitor treatment effectiveness and adjust the treatment plan as necessary to achieve established functional goals. Document all evaluative and therapeutic interactions in the Electronic Medical Record in compliance with department procedures, ensuring clear communication accessible to the care team. Recommend and procure appropriate assistive devices for residents, providing thorough training on their use. Educate residents and caregivers, documenting the outcomes of educational sessions. Collaborate with Physical Therapist Assistants to ensure comprehensive resident care. Understand and adhere to Medicare and other reimbursement regulations, ensuring accurate documentation to meet compliance standards. Provide in-service training on clinical knowledge and skills to nursing and other departments while maintaining current professional competency through ongoing education. Contribute to the development of new rehabilitation programs and services. Stay informed about professional issues through memberships in organizations and relevant publications, sharing insights with colleagues. Supervise assistant-level staff appropriately, ensuring compliance with state mandates and professional standards. Mentor new staff during their orientation, ensuring completion of departmental checklists and competencies. Maintain cleanliness and organization in office and clinical areas, following all infection control procedures. Maintain required licenses, certifications, and competencies, completing annual compliance courses on time. Perform additional duties as consistent with the role.Job Knowledge and Skills Strong verbal and written communication skills. Ability to work collaboratively as a supportive team player. Excellent interpersonal and customer service abilities. Analytical skills with sound decision-making capabilities. Proficiency in Microsoft Office. Exceptional time management and documentation skills. Compassionate, resident-centered care approach. Fluency in English, including reading, writing, and comprehension.Education and Experience Bachelor's degree required; Master’s or Doctorate preferred. Active Physical Therapist license in the applicable state. Basic Life Support (BLS) certification preferred. Preferred experience in a long-term care or skilled nursing facility.Working Conditions/Physical DemandsThe role requires physical and mental capabilities to perform tasks commonly associated with the position, including extended periods of sitting, standing, walking, bending, stooping, and reaching. Candidates must be able to lift up to 50 pounds; lifting over this amount requires assistance.
