Duties include but not limited to assisting with admission procedures, training unlicensed staff, providing medication, treatment administration, and personal care.The LPN performs a variety of professional nursing duties in the care of home care clients, complies with state rules & regulations, and follows established policies and procedures as necessary.This position will be a direct support to the RN. Duties may include the supervision, training and monitoring of unlicensed staff, conducting focused assessments, med passes, auditing, charting, prescreens, admissions and intakes, working with Doctors and Insurance companies in transcribing and ordering medications under the direction of the RN, communicating with clients and families, assist with wound care, skin checks, and other cares as needed. This position is independent in nature, which requires utmost accuracy and discernment in the ability to follow direction and take initiative.Must be comfortable in the supervision and training of all unlicensed staff as they will be passing Meds under your direction. Experience in Memory Care and familiarity with Point Click Care is highly desired.ResponsibilitiesWorks under the direction of the RN to monitor clients' needs and changes in condition, conducting focused assessments when appropriate and reporting the results to the RN Ensures that unlicensed staff members perform their duties and responsibilities in a timely, quality manner, and reports problems to RNReports changes in client condition or other concerns about clients to the RN in a timely manner Initiates emergency procedures for clients and reports to RN Reviews client care plans developed by the RN and consults with RN about client conditions and their treatmentsAssists with implementation of physician ordersCarries out proper charting practices and documentation proceduresAssists with initiation of services for new clients, transfers to settings such as hospitals, nursing homes or assisted living and discharges as requested by RN SupervisorAssembles and operates equipment such as oxygen suppliers and other equipment used for client care Orders, sets up and administers medications Assists RN in orienting, training and mentoring new staffAttends required meetings, in-service training and conferences Performs delegated tasks and other duties as assigned by the RN Assists the RN in the monitoring and evaluation of the job performance of unlicensed staff Schedules and assists work of unlicensed staff within established guidelinesPosition QualificationsLicensed Practical Nurse with current licensure with the State of Minnesota.Current CPR certification.1 or more years experience in Long-Term Care highly preferred Interest in gerontological nursing, rehabilitative and restorative clinical practices.Trained Medication Aide (TMA) a plus.PointClickCare experience preferred.Must be knowledgeable about validation techniques, reality orientation and/or appropriate diversional activities for clients with DementiaMust receive a "Not Disqualified" criminal background result from the Department of Human Services and may not be on the OIG Exclusions ListMust be screened for TBJob Posted by ApplicantPro
