Skilled Nursing of Philadelphia - This position is for a Social Worker in short-term and long-term care nursing home setting.We are looking for a dedicated, compassionate, and emotionally intelligent case worker to oversee and ensure that the medically related emotional and social needs of our nursing home residents are met and maintained on an individual basis. The social worker will work with residents in our nursing home aiding in the access of services and resources to meet those needs while collaborating with our interdisciplinary team.Responsibilities:Interview residents and families to obtain social historyMaintain written documentation in the resident medical record per the company's policy.Develop social service component of the resident's plan of care identifying specific problems, goals, and approaches.Involve residents and families in planning social service programs when possibleCollaborate with interdisciplinary team dailyInterpret social, psychological, and emotional needs of the resident, and family to the medical staff, attending physician, and other resident care team members.Provide solutions for social and practical environmental problems including seeking financial assistance, discharge planning, collaboration and referrals with community agencies as neededProvide support, education and resources to residents and familiesSkillsProven work experience as a Social WorkerWorking knowledge of social theories and practicesSocial perceptiveness and empathyAbility to build and maintain professional helping relationshipsAbility to relate and communicate with diverse population and groupsResilience along with ability to assess situationsQuick thinking, organized, reliable & caringWilling to submit to background checksBS degree in social work (BSW)LSW required$1000.00 Sign On Bonus#IND123
