2nd Shift LPN/RN Supervisor – Set Monday–Friday Schedule + On Call Every 4th Weekend!

At Heritage Pointe, we’re looking for a skilled and compassionate LPN or RN to join us full-time as a second shift supervisor. This role offers a set schedule of Monday-Friday from 2p to 10p with on call duties every 4th weekend.
As a successful candidate, you will provide assistance and care for residents, supervise other nursing staff, offer guidance and support, maintain a clean and safe working environment, and ensure that all operational tasks are completed efficiently and on time. This is a GREAT opportunity if you are just starting in the field, or have plenty of experience.
Key Responsibilities:
Provide skilled nursing care to residents according to their individualized care plans
Administer medications and treatments with accuracy and attention to detail
Conduct daily assessments and document changes in residents’ conditions
Communicate effectively with physicians, residents, families, and team members
Provide direction and supervision to CNAs and other care staff
Ensure compliance with all state and federal regulations
Qualifications:
Current, active RN/LPN license in the state of Indiana
CPR certification (BLS required)
Experience in long-term care or geriatric nursing preferred
Strong clinical judgment and excellent communication skills
