POSITION SUMMARYThe LPN Clinical Care Leader ensures high-quality clinical operations within the Assisted Living facility, alignedwith state regulations and company standards. This role supports resident well-being through effective clinicaloversight, interdisciplinary collaboration, and staff mentorship. The position functions in close coordination withthe Licensed Assisted Living Director and/or Clinical Nurse Supervisor to ensure optimal resident outcomes andstaff satisfaction.ESSENTIAL RESPONSIBILITIES AND DUTIESRESIDENT CARE AND SUPPORT• Support admission processes and ensure tenant-centered service initiation.• Collaborate on resident Service Agreements and Service Schedules in line with clinical assessments.• Assist with focused health assessments and clinical data gathering; communicate findings to the RN andproviders in compliance with 144G and 4659 statutes.• Manage medication tasks including administration, reconciliation, reordering, storage monitoring, andproper medication destruction.• Conduct and monitor wound care, INR checks, pacemaker assessments, and skin integrity measures.• Participate in provider rounding, care plan updates, pain monitoring, and infection control efforts.• Support nutritional management by collaborating with dietary teams.• Promote safe environments through fall prevention, emergency preparedness, and risk managementparticipation.DOCUMENTATION AND COMPLIANCE• Maintain and update accurate clinical records, ensuring alignment with HIPAA and state documentationstandards.• Review documentation prepared by Unlicensed Personnel (ULPs) to ensure it reflects individualizedresident care plans.• Utilize electronic health records proficiently and support documentation compliance.STAFF OVERSIGHT AND DEVELOPMENT• Supervise and support ULPs and other care staff to ensure effective care delivery.• Provide input for performance reviews and assist with staff evaluations.• Promote open, non-retaliatory communication and foster a culture of transparency and safety.• Implement onboarding and in-service training in collaboration with RNs; assist with health education andwellness initiatives.• Coordinate with department directors to ensure competence and accountability in team leadership.FACILITY LEADERSHIP AND QUALITY INITIATIVES• Champion Monarch Healthcare's mission, vision, and values within day-to-day operations.• Drive Quality Assurance and performance improvement through audits, resident satisfaction monitoring,and strategic staff engagement.• Ensure continuous survey-readiness and respond to inspections with leadership and professionalism.• Participate in care and strategy meetings with families and leadership.QUALIFICATIONSEducation and Experience• Current Minnesota License as LPN• Demonstrated knowledge of aging populations and community health resources• Minimum 2 years' experience in Assisted Living or Skilled Nursing• Experience leading teams and staff developmentPreferred• Education or coursework in healthcare administration or leadershipRequired Skills and Abilities• Leadership; achieve established goals through inspiration and loyalty of staff• Business acumen: understands intertwining of departments and how resulting financial metricsdemonstrate their effectiveness.• Resilience: ability to welcome constructive feedback, course correct and not take it personally• Motivator: ability to ensure staff feel supported while ensuring accountability to work expectations• Personability: ability to get along with all personality types and inspire trust with residents, staff, familyand the community• Ability to actively listen, with the goal of understanding• Ability to clearly speak and proficiently read and write EnglishAbout Monarch:Monarch Healthcare Management is a mission-driven company that is changing the way short-term rehabilitation and long-term care are delivered.
