Shift schedule (days, start & end times): Monday through Friday 8:00am - 4:30pmMilage will be reimbursed between visits, from first patient to last patientResponsibilitiesEvaluates patients' functional status and all required body systems to meet CMS documentation standards, and identifies physical therapy needs in collaboration with the physician to develop the plan of careDelivers physical therapy services within state scope of practice, following the established plan of care and working collaboratively with the healthcare teamMonitors and documents patient response to treatment, promptly reporting changes in condition to the nurse supervisor and/or physicianTreats patients to relieve pain, restore or improve function, and maintain maximum independence using evidence‐based practicesEducates patients, families/caregivers, and care team members on the plan of care, functional limitations, safety considerations, and progress toward goalsParticipates in staff conferences, meetings, and committees as requested by the agencySupervises home health aides when permitted by state regulationProvides clinical supervision of Physical Therapist Assistants in accordance with state regulations and agency guidelines.Type of certification required:Current Physical Therapy licensure in state of Washington or compact license granting privilege to practice in the stateCurrent CPR certification
