Make a meaningful impact through your work and help shape the future of Palliative Care in Wisconsin!Qualifications:Board-certified Hospice & Palliative Care Physician Minimum 5+ years of post-fellowship experience Ability to obtain a Wisconsin license This Medical Director role supports inpatient, outpatient, and home-based community visits across Fond du Lac, Ripon, Waupun, Baraboo, Columbus, Madison, Janesville, and Monroe. The position partners collaboratively with 2 Physicians, 11 Advanced Practice Providers, and 15 dedicated support staff, including Registered Nurses, Social Workers, Chaplains, and Medical/Scheduling Office Assistants, delivering comprehensive, patient-centered care across the Wisconsin Region.Job Details:.2 FTE Palliative Medical Director duties, .2 FTE Hospice Medical Director duties, and .6 FTE dedicated to Palliative Care Outpatient and Telemedicine CareCollaborating provider for Hospice & Palliative Care APPsLead program initiativesSet quality and safety metricsInterdisciplinary team – Collaboratively work with Administrative Leaders, Clinical Staff, and Service LinesHelp develop and grow the Regional Hospice & Palliative Medicine ProgramBenefits:SSM Health provides a comprehensive benefits package that includes medical, dental, vision, and pharmacy benefits plans tailored to meet the healthcare needs of you and your family.
