Purpose
The Health-Related Social Needs (HRSN) Coordinator is responsible for ensuring that the agency has the tools, resources, and systems in place to effectively manage the rollout and delivery of the HRSN benefit to clients. This position acts as a connector, ensuring strategic integration within the agency, and fosters collaboration with community-based organizations. The role also involves supporting insurance navigation, overseeing billing processes, fulfilling member needs, monitoring data quality, and managing compliance.

Responsibilities
The Health-Related Social Needs Coordinator will be responsible for the following:
Ensure the agency has systems and resources to manage the Health-Related Social Needs benefit for clients.
Develop, implement, and evaluate strategies to improve health outcomes for eligible members.
Serve as a content expert on HRSN best practices and provide internal guidance to staff.
Collaborate with community-based organizations and providers to improve access to HRSN for members.
Support insurance navigation and ensure accurate and timely billing of services.
Manage compliance programs, including fraud, waste, and abuse prevention.
Maintain accurate records, documentation, and productivity reports; oversee the preparation and submission of monthly invoices.
Develop communication materials, including presentations and reports, to educate and report on HRSN programming.
Supervise and support a team to meet strategic goals, provide training and development, and ensure adherence to agency standards and policies.
Monitor financial audits and data quality, ensuring compliance with internal and external guidelines.
Qualifications
The Health-Related Social Needs Coordinator must possess the following qualifications:
Strong understanding of the impact of generational poverty and social determinants of health on individual outcomes.
General understanding of medical and psychosocial challenges, with a deep knowledge of health effects related to poverty, discrimination, and trauma.
Experience in program management, insurance navigation, and billing processes.
Ability to lead teams, with at least 2 years of supervisory experience preferred.
Strong communication and leadership skills, with the ability to motivate and develop team members.
Knowledge of health equity, health literacy, and shared decision-making principles.
Experience working with community-based organizations and leading collaborative partnerships.
Ability to maintain accurate documentation, track progress, and ensure compliance with security standards.
Valid driver’s license, proof of insurance, or access to reliable transportation and insurance for business travel.
Working Conditions and Physical Effort
The employee must be able to work well in an office environment. Due to the nature of the work, the employee may be required to deal with angry, confused, and/or frustrated clients.
The employee must frequently stand, sit, lift, and/or move up to 10 pounds, and occasionally lift and/or move up to 35 pounds.
Requires considerable walking (including climbing stairs).

Pay: $61,000.00 - $68,000.00 per year
