Home Health Nurse (RN)

Equity Staffing is seeking an experienced Hospice Registered Nurse Case Manager (RNCM) to join a compassionate care team supporting patients and families throughout the hospice journey. This role focuses on providing skilled nursing care, symptom management, patient education, and emotional support in a home hospice setting throughout the Flagstaff, Arizona coverage area.Position Details:Location: Northern Arizona Hospice - Flagstaff, AZSchedule: Monday-Friday, 8:00 AM-4:30 PMKey Responsibilities:Provide hospice nursing care and case management for assigned patientsPerform symptom management and medication managementComplete hospice admissions, recertifications, PRN visits, and symptom follow-upsEducate patients and families on disease progression and the dying processDeliver compassionate personal care support and emotional guidanceCoordinate care plans with interdisciplinary hospice team membersMaintain accurate and timely clinical documentationRequired Qualifications:Active Registered Nurse (RN) license in the state of ArizoniaPrevious hospice nursing experience requiredStrong clinical assessment and communication skillsAbility to work independently in a home health/hospice environmentPreferred Qualifications:Experience with Homecare Homebase (HCHB/EMAR)Compassionate bedside manner and strong family support skills
