Follows up with patients in post-acute environments to ensure compliance with discharge plans post-hospitalization. Performs intake and psycho-social assessments on patients and families and provides psychotherapy to patients and families coping with substance use and behavioral health disorders. Facilitates age-appropriate therapeutic groups and provides family therapy as indicated for patients. Conducts daily clinical rounding and individual therapy sessions for patients. Evaluates for potential safety risks appropriately and responds to potentially dangerous situations to ensure patient safety in the least restrictive manner. Develops and implements appropriate individualized treatment plans based on assessments. Collaborates with the multi-disciplinary team in establishing and documenting psycho-social determinants to ensure safe discharges and completion of discharge plans. Performs discharge planning and post-acute care coordination. Maintains comprehensive community resources and networks to assist patients and families as they transition back to the community. Participates in the development of programs to support patients and families dealing with substance use and behavioral health disorders. Other duties as assigned. Demonstrates working knowledge of the involuntary commitment process.
