By collaborating with healthcare providers, the interdisciplinary team, and care associates, you will fulfill a crucial role in delivering compassionate physical and psychosocial support to residents and their families.What we offerCompensation: commensurate with experience, $36 -$42 an hour plus shift differentials- $2.50-eves $3.00-nights and $3.50 for all shifts on the weekend!$7,500.00 sign on bonus for both full and part-time.A culture of diversity, inclusion, equity and belonging, which builds on our mission, vision and valuesMedical, dental and vision packages, including an annual reimbursement for qualified wellness expenses, personal health coaching and telemedicine optionsPTO Plans, PLUS company paid volunteer hours for eligible team members, in accordance with applicable state law401k for all team members 18 and over with a company 3% matchOnsite medical centers, providing wellness visits and sick care for all team members over 18 years of ageFree access to our on-site Team member Health and Well-Being Centers, plus Well-Being programs, tools and resources for you and your immediate family membersEducation assistance, certification reimbursement and access to over 6,000 courses through our online learning library, designed to enhance your current skills and build new onesGrowth Opportunities – grow with the company as we open new communities and expand on our existing ones!.How you will make an impactDeveloping relationships with the residents and their families using a consistent caregiver approach supporting the resident's physical, spiritual, emotional, and clinical needs and focusing on placing the resident first.Assessing, observing, and evaluating resident's clinical condition, document findings in the resident electronic medical record, and notify the provider and/or RN when necessary.Updating or modifying care plans as needed to reflect any changes to a resident's care and service needs.Supervising and leading care associatesAdministering medications timely, accurately, and according to orders.Participating in meetings related to resident outcomes and careResponding to residents, family, or responsible parties in a timely and professional mannerParticipating in the review of clinical indicator reviews – falls, wounds, nutrition, antipsychotic use, etc.Other duties as assigned by RN clinical leaderWhat you will needMinimum of 1 year of experience in assisted living, home health, or long-term care is preferred.Licensed by State as a Licensed Practical Nurse.CPR certification required.A passion to care for seniors and be a strong resident advocate.Experience with electronic medical records is preferred.Please note that specific state regulations and requirements may be applicable.
