The Care Coordinator conducts assessments, coordinates home and community-based services and assists members in making progress towards his or her health goals while coordinating care with the interdisciplinary team. The position involves both remote work and requires home visits to conduct in person assessments.Education QualificationsThis candidate will need to live in the Moorhead, MN area so they can visit patients in the Moorhead area.
