Job Description & Requirements Specialty: Hospice Discipline: RN Start Date: 06/08/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Simplifi Clinical Interviews - Manager to review any offers Unit: Hospice Case Manager Experience Required: 3 years of Home Hospice or Home Health experience; will accept first time travelers Certs Required: BLS, Drivers License, Auto Insurance, Vehicle Skills Required: Wound care, dressing changes, wounds vacs (wounds must be photographed and measured weekly), drains (Pleurex, nephrostomy, foley) care coordination, end of life care/planning, pain/symptom management, triage, education to patient/family. Soft skills: being organized, time management. Floating: Coverage territory includes, but is not limited to, the town of Lynwood, Calumet, Calumet City, Calumet Park, Riverdale, and Dolton Patient Types: All Adults. Failure to thrive, advanced cancers, end stage COPD, end of life, terminally ill, end stage disease processes Total Case Load: 12-15 Scheduling: Weekend rotation: No weekends On call?
