Job Description Job SummaryWe are seeking an experienced Home Health Physical Therapist Assistant to join our team in the Houston and/or surrounding areas! As an Physical Therapist Assistant, you will be providing therapy and support for our patients in their homes. You will be following plans of care previously laid out, assisting your patients with using adaptive devices, and maintaining all therapy spaces and equipment. Responsibilities Read, understand, and follow all plans of care and therapy laid out for patients under your careAssist patients with using adaptive equipment, modified exercises, and learning basic life skillsTimely completion of progress notesCommunicate with the Physical Therapist as neededQualificationsCurrent state licensing for physical therapy assistant requiredPrevious experience working in home health desiredFamiliarity with Kinnser is helpfulCPR certifiedProfessional Liability InsuranceCurrent Drivers LicenseAuto Insurance
