About the Opportunity:The Associate Discharge Planner supports the care management team by coordinating patient discharge planning and assisting with departmental operational functions. This role ensures efficient transitions for patients from hospital settings to home or alternative care environments, directly impacting the quality and continuity of patient care.3. Responsibilities:• Coordinate discharge planning activities to facilitate smooth patient transitions• Assist with operational functions within the care management department• Collaborate with multidisciplinary teams to ensure discharge plans meet patient needs• Support documentation and communication related to discharge processes4.
