LPN/RN - PCP (Float)

Under the direct supervision of the clinician, department supervisor or designee, assists in providing clinical care to office patientsManages clinicians' daily schedule and patient flow to expedite care and maximize clinical timeGreets and escorts patients to exam roomsObtains relevant information and takes vital signs as indicated (i.e. height, weight, blood pressure, heart rate, respiratory rate, oxygen saturation).Inputs pertinent clinical information into the electronic health record timely and with accuracy and performs a complete medication reconciliationAssists clinicians with clinical procedures and/or additional tests specific to the department. May advise patient of preparation required for exam or additional testing and keeps patients informed of visit status, unforeseen delays and other relevant information pertaining to the patient's visitPrepare and maintain exam rooms for patient arrivals. Clean and/or maintain inventory of supplies (medical included) and equipment throughout the practice. Notifies supervisor to order clinical supplies, clinical equipment and pharmaceuticals as appropriateUnder the supervision of a physician, administers routine injections when delegated, and documents in real time and with accuracyEnsures telephone calls are answered timely and clinicians receive necessary informationCoordinates care when clinicians are not immediately available, ensuring that telephone calls are triaged appropriately per established protocolsAssists in resolving patient concerns by acting as an advocate, as appropriateParticipates in practice and department meetings, trainings, etc. to promote continuous improvement and quality patient careTakes initiative to learn, understand, and stay current with community resources and the greater healthcare system while continuously participating in self-assessment of skillsMaintains patient rights and abides by all confidentiality obligations pursuant to CCHC and HIPAAProvides clerical/office support as necessary (i.e. scheduling appointments, greeting and registering patients, scanning incoming medical records and paperwork, preparing medical records, answering phone calls and taking messages, faxing, etc.)Ability to float to other MACC practices when requiredPerforms other work-related duties and activities as assigned or requestedAssists with the assessment of patient needs and coordinates care under the direction of the clinicianVerifies and implements clinician's orders for testing and/or procedures (EKG's, dressing changes, spirometry, Holter monitors, etc.)Provides effective and evidence-based patient and family teaching, using health promotion and health teaching methods in collaboration with the patient's values, beliefs, health practices, developmental level, learning needs, readiness and ability to learn, language preference, spirituality, culture and socioeconomic statusTriages walk-in patients per established protocolsMaintains communication and collaboration with interprofessional team and others to facilitate safe transitions and continuity of care deliveryLPN (P016) - EDUCATION/EXPERIENCE/TRAINING:* Ability to read, write and communicate in English* Graduate of an accredited Practical Nursing program* Currently licensed as a Licensed Practical Nurse in the state of Massachusetts* Basic Life Support Certification for Health Care Providers* Strong interpersonal, customer service, time management skills* Experience with current office technology and proficient with computer office applications (Electronic Medical/Health Records, MS Office, etc.)* A Minimum of two years' experience in a clinical setting preferredRN (P021) - EDUCATION/EXPERIENCE/TRAINING:Ability to read, write and communicate in EnglishGraduate of an accredited school of nursingCurrently licensed as a Registered Nurse in the Commonwealth of MassachusettsBasic Life Support Certification for Health Care Providers* Experience with current office technology and proficient with computer office applications (Electronic Medical/Health Records, MS Office, etc.)* Minimum of two years' experience in a clinical setting preferred
