The RN Care Manager is an integral member of the primary care team, working collaboratively with providers to engage patients in improving chronic disease self-management, coordinating care, and reducing unnecessary healthcare utilization.In this role, you will support high-risk patients with complex medical and social needs by coordinating care across multiple providers and settings, providing chronic disease education, performing medication reconciliation and education, and helping patients navigate the healthcare system. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
