Physical Therapist All Heart Home Care | Nashville Area | PRNAll Heart Home Care is seeking a compassionate and skilled Physical Therapist (PRN) to join our home health team in the Nashville area. This role offers flexible, as-needed scheduling while providing personalized, one-on-one care to patients in the comfort of their homes.Key Responsibilities:Perform in-home evaluations to assess patient needs and develop individualized treatment plansProvide skilled physical therapy services to improve mobility, strength, balance, and overall functionMonitor patient progress and adjust treatment plans as appropriateEducate patients and caregivers on home exercise programs, safety, and injury preventionComplete accurate and timely clinical documentation in compliance with company policies and state regulationsCoordinate care with physicians, nurses, case managers, and other members of the healthcare teamTravel to patient homes within assigned service areasQualifications:Doctor of Physical Therapy (DPT) or equivalent from an accredited programActive Physical Therapist license in the state of TennesseeCurrent CPR certificationValid driver's license, reliable transportation, and current auto insuranceStrong clinical assessment and communication skillsAbility to work independently and manage time effectivelyPrevious home health experience preferred but not requiredWork Schedule:PRN (as needed)Flexible scheduling based on patient volume and availabilityAbout All Heart Home Care: At All Heart Home Care, we are committed to providing high-quality, compassionate care to patients in their homes.
