Our team of specialists works together to guide each patient through every step of the cancer experience—from diagnosis through survivorship.Position OverviewThe Oncology Nurse Navigator serves as a dedicated care coordinator throughout the cancer care continuum. This role assesses and addresses the physical, emotional, and social needs of patients, while working closely with healthcare providers and internal/external partners to remove barriers to care. ONNs provide education, improve access, and streamline communication to ensure a supportive, seamless experience for patients and their families.Key ResponsibilitiesAssess, plan, facilitate, and coordinate care to meet patients’ physical, emotional, and social needsAdvocate for timely, high-quality care by guiding patients through the healthcare systemCoordinate appointments, referrals, and services to ensure continuous careProvide education on treatment plans, symptom management, and health maintenanceMaintain accurate documentation and tracking of patient progress and clinical outcomesReduce barriers to care through anticipatory guidance, insurance coordination, and community resource navigationCollaborate with oncologists, specialists, and support teams to enhance patient experience and outcomesEducate patients on individualized care plans to reduce unnecessary utilization and improve quality of lifeServe as a trusted resource for patients and families throughout the cancer journeyPerform RN scope duties, including telephone triage and clinical assessmentsParticipate in clinic activities and assist in care coordination and planningPerform other duties as assignedQualificationsEducation & ExperienceGraduate of an accredited nursing program (required)At least 1 year of nursing experience (required)Bachelor’s degree in nursing (preferred)Outpatient nursing and scheduling experience (preferred)Credential/LicensureCurrent RN license in the state of practice or compact license (required)BLS certification through the American Heart Association (required)OCN certification required within two (2) years of hireSkills & AbilitiesStrong knowledge of oncology practices and patient navigationExcellent verbal and written communication skillsCritical thinking and conflict resolution skillsAbility to prioritize, manage time effectively, and work both independently and collaborativelyProficiency with electronic documentation and EMR systemsCompassionate, patient-centered approach with strong attention to detailWhy West Cancer Center is a Great Place to WorkMeaningful Impact: Play a direct role in supporting patients through one of the most challenging times of their lives.Collaborative Culture: Work alongside a multidisciplinary team of dedicated professionals committed to improving cancer care.Professional Development: Benefit from ongoing training, educational resources, and growth opportunities.Mission-Driven Environment: Be part of an organization guided by compassion, integrity, and innovation.No Nights, Weekends, Or Holidays.
