Family Practice - Geriatrics Physician

Job Title: Family / Internal / Geriatric Medicine Physician Location: Forest Lake, MN Shift: Full-time & Part-time position Summary: We are looking for a Physician to serve patients in Long-Term Care (LTC) and Transitional Care Units (TCUs) . Responsibilities : Conduct comprehensive patient assessments and ongoing medical evaluations. Create and manage personalized treatment plans based on patient health needs. Diagnose and treat acute and chronic conditions , providing timely medical interventions. Prescribe medications, order diagnostic tests , and modify treatments as needed. Work closely with nurses, therapists, and social workers to coordinate comprehensive care. Requirements: MD or DO degree from an accredited medical school. Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatrics preferred. Valid medical license to practice in Minnesota. Previous experience in long-term care, transitional care, or geriatrics is a plus. Excellent problem-solving, communication, and teamwork skills. For more info, please reach me at / .
