Part Time Family Support Service NavigatorLocation: MCS Holyoke (260 Westfield Rd, Holyoke)Responsibilities: Service Navigators support families who have a family member with an Intellectual Disability. Supports include, but are not limited to case management, accessing generic services, assessing needs, developing Family Support Plans, and providing information and referrals. Must have valid driver's license and dependable vehicle.Hours: 20 hours per weekSalary: $23.50/hrMaintains quality of programming through adherence to agency standards and DDS regulationsService Navigation for the Children, Transition & IFFSWork closely with DDS Service CoordinatorsMaintain accurate financial recordsOutreach to generic services and other state agenciesEnsure that all documentation is completed on a timely basis, and other duties as requiredRequired Trainings/Skills: Experience working with individuals with intellectual disabilities and Bilingual in (English/Spanish) preferred but not required.For more information about MCS please visit mcsnet.org
