They also provide home medical equipment and supplies, empowering clients to live independently while maintaining health, safety, and well-being.Pay: $43.00 - $46.00 per hourPosition Description: Self-Direct Inc. (LHCSA) is currently seeking a per-diem RN Supervisor, responsible for supervision of clinical operations and maintaining compliance with all federal, state and LHCSA regulations governing Home Health Care. They are also responsible for training and supervision of personal care aide clinical activities, development and implementation of patient care plans.Self-Direct, Inc. is committed to providing quality care and quality of life for the patients and families we serve.REGISTERED NURSE (RN) DUTIES/RESPONSIBILITIES:Provides medical oversight for clients in their homes- including but not limited to on-site field visits, patient assessments, obtaining MD orders, and maintaining an appropriate Plan of CareCommunicate with discharge planners and physicians to ensure seamless patient care.Develop, implement, and monitor individualized treatment plans, ensuring proper care coordination and medication management.Perform in-home patient assessments, reassessments, and provide intermittent nursing services.Ensure accurate documentation and compliance with agency policies, including patient rights and discharge planning.Supervise and provide in-service education for staff, conducting competency evaluations.Participate in quality improvement programs and policy reviews.May assist with PCA Training Program as NeededBe responsible for direct supervision of field PCA staff. Coach staff and recommend in-service education programs and ensure adherence to internal policies/standards.Ensure service quality and participate in care coordination to ensure proper communication between caregivers, patients, referral sources, and payers. Discuss operational issues, update staff on new/changed regulations, and review records/documentation to ensure regulatory and in-house compliance.Participate in performance improvement activities, maintain ongoing clinical knowledge through internal/external training programs, provide interpretation of knowledge and direction to staff.Qualifications:Current NYS RN licenseThorough knowledge of the scope of practice of professional and paraprofessional healthcare.1 year Home Care experience, preferredValid driver’s licenseStrong interpersonal, verbal, and written communication skills requiredComputer literacy, including but not limited to data entry, retrieval, and report generationAt Self-Direct, Inc. we are committed to providing a safe and secure work environment by adhering to guidelines set forth by the New York State Department of Health, Federal agencies, and the Centers for Disease Control and Prevention (CDC) in relation to COVID-19.
