We are seeking a compassionate *APRN* to provide patient-centered care in skilled nursing and rehab settings across Texas, diagnosing, treating, and managing acute and chronic conditions while collaborating with interdisciplinary teams.Deliver direct care, perform assessments, develop treatment plans, and prescribe medications.Accurately document all care in EMR/EHR systems and support admissions/discharges.Contribute to reducing hospital readmissions and ensure compliance with infection control and regulatory standards.Qualifications include an active *TX APRN license*, BLS/ACLS certifications, and at least 1 year of relevant experience.This part-time role offers competitive pay ($120,000 - $180,000 annually) with ongoing education, clinical support, and documentation tools.
