Wellness Director, FT Days

Brief DescriptionGENERAL SUMMARY:Under the administrative supervision of the Administrator, the Wellness Director is responsible for the health, safety and welfare of the residents by supervising and directing the care given by the Personal Care Attendant, ensuring that care is provided to residents in a respectful, caring and professional manner in accordance with regulations and established policies and procedures. The Wellness Director must conduct him/herself in a professional manner while responding to families, visitors, staff and volunteers.EDUCATION:Graduate of an accredited Nursing Program required.WORK EXPERIENCE:Minimum Of One (1) Year Supervisory Experience Preferred.Two (2) years of satisfactory experience in a hospital/rehabilitative setting preferred.Two (2) Years Assisted Living/Personal Care Experience Preferred.Monitors and assesses health status of residents. Assesses appropriateness of level of health care services and provides services based on assessment including medication administration, assistance with activities of daily living or referral for different level of care. (E)Directly provides professional nursing services including admission and ongoing nursing assessment of residents, first aid, and emergency care. (E)Communicates assessments and recommendations for adjusting levels of care to the physician and Administrator. (E)Assigns, coordinates and supervises resident care given by Personal Care Attendants. (E)Charts and records all relevant and necessary notes, comments, observations and events in appropriate logs, reports, care record, summary sheets, etc. Receives and provides reports to other staff at appropriate times. Reports to the Administrator of Allied Terrace any errors, discrepancies and unusual happenings or events. (E)In consultation with the Administrator, instructs, counsels, evaluates and disciplines employees. (E)Monitors individual work performance through observations, spot-checking, work completed and conferences. Promotes individual staff development. Counsels with staff regarding performance problems. Monitors personal care records to assure that meds, care and treatment are being provided as ordered. (E)Insures that all health care services provided are in compliance with the state and federal regulations for the provision of such services. (E)Responds to both emergency and non-emergency calls according to established policies and procedures. Calls 911 or physician as dictated by the situation. Completes the official transfer sheet and all other required information according to procedures and faxes to the hospital to which the resident was admitted before reporting off duty. Notifies the party responsible for the resident’s medical decisions. Documents all actions taken. (E)Prepares work schedules for staff including mealtimes and assigning breaks. (E)Collaborates with Dining Room Supervisor regarding nutritional needs for personal care residents. (E)Maintains current, familiar knowledge of all Allied Services nursing care policies and procedures, fire and disaster action plans, infection control and pharmacy policies, associated services and other nursing-related directives and resource documentation. (E)Attends and completes all required inservice programs and sessions. May contribute to content. (E)Maintains awareness of, and fulfills in a timely manner, employee-related supervisory responsibilities, including accurate completion and distribution of “Employee Confidential Form”, “Employee Performance Appraisal Form”, etc. Ensures that staff is aware of, and follows, Allied Services Human Resources Policy & Procedure Manual as well as the policies and procedures of the Allied Terrace Division. Makes effective recommendations in matters of hiring, promotion, discipline and termination. (E)
