Hospice Nurse Practitioner
Job Summary:
We are seeking a compassionate and experienced Hospice Nurse Practitioner to join our dedicated team. The ideal candidate will be providing clinical assessment, diagnosis, and treatment, as well as managing complex symptoms and medications.

Clinical Assessments: Conduct Comprehensive assessments of patients with life limiting illness, including physical, psychosocial, and spiritual dimensions.
Advanced Symptom Management: Utilize advanced knowledge and skills to manage complex symptoms such as pain, dyspnea, nausea, and delirium.
Medication Management: Prescribe, and adjust medication as needed to control symptoms and minimize side effects.
Interdisciplinary Collaboration: Participate in interdisciplinary care team meetings, collaborating with physicians, nurses, and other health professionals to coordinate holistic care and address the diverse needs of patients, as well as their family.
Patient and Family Education: Provide education and support to patients and their families regarding their illness trajectory, treatment options, symptom management strategies, advance care planning, and end of life preferences.
QUALIFICATIONS
Master's degree in nursing or equivalent
Current license to practice in the state of employment and Controlled Substance.
Active DEA License with no restrictions as required.
One plus year of home care or hospice experience
Ability to collaborate with the patient's primary physician to interpret the hospice program and affect optimum medical care.
