TRANSITION ADMISSION TEAM RN / HOME HEALTH SERVICES

Part-time / 2.5 days per week from 8:00am - 4:00pm & 8:00am - 12:00pm, with on-call & weekend rotation required;Pay range begins at $30.12/hrJOB REQUIREMENTSEDUCATIONMinimum: Graduation from an accredited nursing program; Bachelor degree in nursing science (BSN)Preferred: Bachelor degree in nursing science from an accredited nursing program accredited by a professional nursing accrediting body.LICENSE/CERTIFICATIONRegistered Nurse Licensure from the state of Indiana; CPR required. COS-C preferred.EXPERIENCEMinimum: At least two years Med-Surg experience required and previous Home Health or Hospice experience.Preferred: 1-2 years nursing experience in Home Health and Hospice.
