Wound Nurse Coordinator Full-Time: 8am-430pm Sign-On Bonus: LPN – up to 3 years’ experience $7500, over 3 years’ experience $10,000 RN - up to 3 years’ experience $10,000, over 3 years’ experience $12,500 Purpose The primary purpose of your job position is to direct, organize and develop all wound care services in accordance with applicable federal, state, regulatory and facility standards to ensure the delivery of quality resident care is rendered at all times, and that the Plan of Care and physician orders are followed with regard to wound care treatments. This position functions independently, with oversight by the Director of Nursing (DON) or Assistant Director of Nursing (ADON) demonstrates above average communication skills, personal integrity and works effectively with residents, family members, physicians, staff and outside agencies as required. This position leads wound education and supervises clinical staff in addition to providing direct wound care and treatments. The Wound Care Nurse will deliver care competently to those residents with complex wounds, pressure areas, ostomies or continence needs. The Wound Care Nurse will assist nursing staff in identifying residents at risk of pressure ulcers; define early interventions for prevention and management. This position reports directly to the DON or ADON. Education and Experience 1. Must be, at a minimum, a graduate of an approved RN or LPN program. 2. Certification in wound care Preferred. 3. Must have a minimum of two years’ experience directly related to wound care. 4. All licensed nurses shall possess a valid Healthcare Provider cardio-pulmonary resuscitation (CPR) certificate within six (6) months of their first day on the job in the facility. CPR classes are provided by LMC Extended Care Staff Development. Specific Requirements 1. Must possess a current, unencumbered license to practice as an RN or LPN in this state. 2. If not already obtained, acquired wound certification within 6 months of hire. MAJOR DUTIES AND RESPONSIBILITIES 1. Provide care and treatment for pressure areas, acute wounds, chronic wounds, diabetic ulcers, draining wounds and fistulas, 2. Cooperate with physicians and other staff nurses in the development of treatment of wounds. 3. Implement and carry out treatment as ordered by physicians. 4. Make decisions concerning wound care in collaboration with the DON regarding treatment protocols when necessary. 5. Notify the resident’s attending physician of changes in wounds or sign of infection. 6. Initiate referrals when indicated for further medical or surgical intervention. 7. Requisition and arrange for diagnostic and therapeutic services as ordered by the physician and in accordance with our established procedures. 8. Consult with resident’s physician and responsible party when there is a change in the resident’s condition. 9. Review the resident’s chart for specific treatments, medication orders, diets, etc. as necessary. 10. Maintain required records and reports. 11. Visualize wounds/pressure injuries weekly. 12. Complete weekly Pressure Injury and Non-pressure Wound reports. 13. Maintain reports in preparation for Wound Care meeting. 14. Chair the Wound Care meeting. 15. Supervise and evaluate the treatment nurses. 16. Provide education to treatment nurses and other nursing staff. 17. Attend and participate in the Quality Improvement Committee.
