Role DescriptionTravel to different facilities in a coordinated area to treat Geriatric patientsPrepare and update patient records through our EHR system and publish test results to our online medical care portalTreat patients for internal medicine ailments and order blood work and other tests as neededTravel to patient home and different facilities such as Nursing Homes, Assisted Living to treat patientsEstablish rapport with patients and encourage them to talk with you candidly and franklyProvide advice on healthy habits and practices for patients based on their current health statusPrescribe and refill medications as neededProvide referrals to specialists when requiredAssist practice physician when neededPerform physical examinationsCreate treatment plans Qualifications Strong clinical skills, including patient assessment, health evaluation, and chronic disease managementExperience in prescribing medications, developing care plans, and managing treatmentsProficiency in healthcare documentation, patient records management, and care coordinationExcellent communication skills for interacting with patients, families, and interdisciplinary healthcare teamsAbility to provide compassionate, patient-centered care and adapt to diverse healthcare settingsCurrent Nurse Practitioner certification and valid licensure to practice in FloridaMinimum of 2 years of experience in geriatric or primary care practice preferredMaster’s degree in Nursing (MSN) or related field requiredBLS/ACLS certification preferred
