About the Role:
As a Case Manager, you will play a vital role in supporting clients through discharge planning, legal or employment coordination, and connection to aftercare services. You’ll collaborate closely with clinical staff to provide integrated, client-centered care and make a lasting impact on those in recovery.
Key Responsibilities:
Initiate family contact and complete intake documentation within 72 hours of admission
Coordinate legal matters (probation, court, FMLA, disability forms) and support clients’ employment rights
Develop personalized aftercare plans based on clinical recommendations
Serve as a liaison between clients and external providers (IOP, therapists, psychiatrists, etc.)
Attend treatment team meetings and communicate client needs to leadership
Maintain accurate documentation in EMR
Follow all infection control and ethical guidelines
Qualifications:
High School Diploma or GED required; Bachelor’s degree preferred
Prior experience in healthcare, behavioral health, or social services case management
Strong knowledge of community resources and HIPAA regulations
Excellent communication, time-management, and teamwork skills
EMR proficiency and ability to maintain accurate records
Certifications such as CADC or LSW are a plus

You’ll be part of a collaborative team of counselors, therapists, physicians, and support staff who are dedicated to providing holistic care to adults (18+) with substance use disorders and co-occurring diagnoses.
Work Environment & Requirements:
Indoor, climate-controlled clinical setting
Some walking, sitting, light lifting, and occasional exposure to emotionally intense situations
Minimal exposure to workplace hazards or violence

Pay: $22.45 - $24.66 per hour
