Job Title: Licensed Practical Nurse (Home Health) Private Duty Agency: Altra Home Health Location: Plymouth, IN Position Type: Full-time/Part-time/PRN Reports To: Clinical Supervisor Job Summary Altra Home Health is seeking a compassionate and skilled Licensed Practical Nurse to join our team. In this rewarding role, you’ll provide personalized care to pediatric and adult patients in their homes — many of whom have complex medical conditions, including gastrostomy tubes (G-tubes), cerebral palsy, chronic diseases, and occasionally require tracheostomy or ventilator support. You’ll play a vital role in helping them live safely and comfortably in their home, where they thrive the most. Responsibilities Perform thorough head-to-toe assessments and follow individualized plans of care Provide care which may include G-tube feedings, medication administration, wound care, trach care and ventilator support. Care for patients with professionalism and empathy.
